COVID-19 Vaccine Date Set

To our dearest families and friends,

We are very excited to share with everyone, we have a date set for residents to receive the first dose of the COVID-19 vaccine.

Our scheduled date is set for Saturday February 13,2021 from 10 am- 4 pm. The injections will be administered by CVS pharmacy staff. Below you will find a link to the consent form needing to be filled out and signed by the resident’s responsible party. If you are the resident’s responsible party, please fill out the form and please return via e-mail, mail or drop it off back to the communities receptionist as soon as possible so we can submit the vaccine count to CVS pharmacy.

We are highly encouraging all residents and staff to receive the vaccine as this will give everyone the best protection against the virus that is currently available and will be a significant step in our community eventually returning to a less restrictive set of practices.
