COVID-19 Second Vaccine Dose Date Set

Dear Residents, Families and Friends,

Our first vaccine clinic this past Saturday went off with great success. We vaccinated 106 residents and staff. On behalf of everyone here at the Gardens, I would like to thank the Dexter family for delivering pizzas for everyone. We now have a date set for those who received their first dose at this clinic to receive the second dose of the COVID-19 vaccine. The date for our second clinic is Saturday, March 6, 2021 from 10 AM to 2 PM administered by CVS pharmacy technicians.
For any residents receiving the COVID-19 vaccine at the next clinic, below you will find a link to the consent form needing to be filled out and signed by the resident’s responsible party. If you are the resident’s responsible party, please fill out the form and please return via e-mail, mail or drop it off back to the community as soon as possible.
